CUA Application Instructions

 |
- For all “Authorized Activities,” download application and complete following the instructions given. If you have questions regarding an activity that is not currently listed under “Authorized Activities,” please contact CUA Coordinator David Stransky, at 808-422-2771 X 112.
- Review insurance requirements for Automobile and General Liability Insurance and mail a copy of proof of insurance along with completed application. Make sure USS Arizona Memorial is listed as an “additional insured” on each certificate. A permit will not be issued unless insurance requirements are met.
- Review and understand conditions of permit.
- Complete, sign, and date application Form.
- Mail completed and signed forms and fees. Forward no later than 30 days after receipt to be eligible for the initial CUA qualification determination (see Section IV for address & contact).
- Confirmation and Award: All requests for CUAs will receive a response from our office. If the application results in award, the CUA will be mailed to the applicant for signature. If the entity is not awarded a CUA, the application will be returned along with the submitted fees minus the application fee.
- Annual Reporting, and Company Information: Once awarded, the CUA Operator is required to forward the following:
Brochure or online information site address showing how the activity is described and conducted (if the entity advertises the service).
Annual Report: Annual Report is due upon receipt of CUA.
