Instructions for Completing the Application Form

Please read carefully. Applicants are encouraged to consult with their Regional Coordinator before and during the application process.

The first step in the application process is to complete the cover page providing general information about the element. Then, based on the type of element, complete the appropriate section. If the applicant is applying for inclusion in the Network as a site, then complete only Section A. Applicants who are applying for inclusion as facilities should complete only Section B. All program element types should complete only Section C. Applicants may apply for inclusion as more than one type, but one is sufficient for inclusion. Note that if the element does not meet the criteria for one type, perhaps it meets that of another.

Attach additional pages and supporting information as necessary, but be sure to identify element name and questions addressed on each additional sheet or item of additional material. The application form may be submitted by e-mail, but all application packages, including any supporting material, must be submitted in hard copy to the appropriate Regional Coordinator. See the Network Definition Statement to identify which office represents the applicant’s state. Material sent by regular mail must be postmarked on or before the application deadline to be considered for that review round.

The applicant can submit either original or photocopies of letters of support. All photographs should be submitted in either digital format or 35mm print film, in color or black and white.

Approved applications and all supporting material will become the property of the National Park Service and will not be returned to the applicant.
